Accreditation

CLS has been accredited since 2005 by CARF International, an independent, non-profit accreditor of health and human services. At our 4th site survey in February 2015, we again received a three-year accreditation; the highest result possible. “Include Me!” CLBC Supported Individual Quality of Life Survey

The CLS once again participated in the “Include Me!” Quality of Life Survey in the 2015 - 2016 fiscal year (last completed in March 2011). This survey was facilitated by Malatest on behalf of CLBC, and 144 supported individuals from a variety of service areas were asked to be interviewed in person by trained peer interviewers either by phone, on-line or via two proxies reporting on behalf of the individual. “The survey and the implementation process have been designed to ensure that the voices of individuals with developmental disabilities are truly heard.” CLBC Include Me! The CLS believes that this process captures a general picture of supported individuals overall quality of life from their own perspective by reviewing all of 8 domains in the areas of Independence, Social Participation and Well-being. The information collected in this report will be used to assist the CLS to determine our organization's priorities and assists us to prioritize strategically about the areas we can actively address moving forward.
